- Affiliation Form. The affiliation form asks you to identify your disability or access issue, what accommodations you may have had in the future, and what accommodations you have in mind to address the issue that you are facing now.
- Supporting Documentation. Supporting documentation includes diagnosis(ses) and reference to where/how these diagnoses cause issues in your work at TCNJ. These can be emailed to maldonac@tcnj.edu or uploaded to the form
- Meeting with an Accommodations Manager. After your information is reviewed, you will be contacted to ensure we understand what you need; you have an opportunity to ask any further questions, and to settle on an accommodations plan and its implementation. This process will also include the accommodations manager reaching out to your supervisor to discuss how and if your accommodations are reasonable given your job responsibilities.
